The County of Santa Clara Social Services Agency’s (SSA) Department of Family and Children’s Services (DFCS), invites qualified, persons, firms, entities and organizations (“Proposers”) to submit proposals to provide the following services within and for the County of Santa Clara (“County”):
Transitional Housing Placement – Plus (THP-Plus) Program
THP-Plus is a transitional housing program for emancipated youth or young adults who exited foster care, including those supervised by the Juvenile Probation, on or after their 18th birthday. This program offers housing and supportive services for up to 36 cumulative months or until the age of 25, whichever comes first.
The goal of THP-plus is to provide a safe living environment while helping participants develop the life skills needed to live independently. The program’s goals are achieved by creating a Transitional Independent Living Plan (TILP) with the participant. The TILP is developed by young adults and other supportive persons who assist in identifying and detailing the goals and objectives necessary to transition into adulthood.
