Amendment 3 hereby extends proposal and Past Performance Questionaires to June 18, 2026 at 12:00pm.


Amendment 2 hereby extends the proposal and past performance questionaires due date from June 1, 2026 at 12:00pm EST to June 12, 2026 at 12:00pm EST, extends question period from May 26, 2026 at 12:00pm to June 1 at 12:00pm EST, and updates Attachment 1 OHS Performance Work Statement which updates Section 9.0. 


Amendment 1 hereby updates Attachment 3 Training and Certifications Requirements.


This notice is a combined synopsis/solicitation for commercial products and commercial services prepared in accordance with the FAR Part 12, Acquisition of Commercial Products and Commercial Services. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued.


This notice is being issued as a Request for Quotations (RFQ) under solicitation number 80AFRC26Q0005 for Armstrong Flight Research Center Occupational Health Services.


The provisions and clauses in the RFQ are those in effect through FAC 26-01.


This acquisition is a total small business set-aside.


The NAICS Code and Size Standard are 621498 and $25.5M, respectively. The offeror must state in their offer their size status for this acquisition.


All responsible sources may submit an offer which must be considered by the agency.

Offerors must provide the information required by FAR 52.212-1, Instructions to Offerors-Commercial Products and Commercial Services, (OCT 2025) (DEVIATION). 


FAR 52.212-2, Evaluation -- Commercial Products and Commercial Services, (OCT 2025) (DEVIATION). The Government will award a contract resulting from this solicitation to the offer responsible, whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors are of importance and must be used to evaluate offers:


Technical Acceptability, Relevant Experience and Past Performance, and Price are approximately equal. When combined, Technical Acceptability and Price are more important than Relevant Experience and Past Performance


FAR 52.212-4, Contract Terms and Conditions-Commercial Products and Commercial Services, (OCT 2025) (DEVIATION)  is applicable.
