The Arizona Office of Economic Opportunity (OEO) has launched the Rural Health Transformation Program (RHTP) under solicitation CMS-RHT-26-001 to address healthcare workforce shortages in rural and tribal communities. This cooperative agreement specifically targets public universities to expand training capacity for high-demand health occupations, bridge geographic gaps in specialized care, and ensure long-term workforce retention. The program utilizes two pathways for learners: subsidized students who commit to a five-year service obligation in eligible rural areas, and non-subsidized students who receive targeted career services to encourage voluntary rural placement. Performance is measured by the number of rural learners trained, workers placed, and workers retained within rural health systems. The program is structured across five budget periods from fiscal year 2026 through 2030, with an initial 10-month period followed by four 12-month periods. Funding is awarded via a proportional formula based on a points-based evaluation of technical initiatives, data-driven outcomes, and state policy actions. Applicants must adhere to strict regulatory requirements, including 2 CFR 200 compliance, the Stevens Amendment, and non-supplantation of funds. Key constraints include a 10% cap on administrative costs and the prohibition of funding for clinical services already reimbursable through insurance or independent research activities. Universities are responsible for verifying that participants physically provide services in designated rural areas, as telehealth-only service does not satisfy the rural service commitment.

Program Overview The Arizona Office of Economic Opportunity (OEO) is pleased to announce the launch of the Rural Health Transformation Program (RHTP). This initiative is a strategic response to meeting Arizona's health workforce needs in rural and tribal communities. With the healthcare sector projected to add 76,000 jobs by 2030, the RHTP serves as a cornerstone for economic mobility and rural resilience. Our mission is to transform rural healthcare delivery through targeted workforce expansion and system coordination. This grant program specifically prioritizes partnering with public universities to bolster healthcare professions in rural and tribal communities. Program Goals By providing funding to Arizona's universities, OEO aims to:
- Expand Training Capacity-Increase the number of rural learners entering high demand health occupations in rural communities, including allied health professions, Graduate Medical Education (GME), accelerated physician training models, and health professions with serious specialty shortage designations. - Bridge Geographic Gaps - Facilitate clinical rotations and residencies in rural and tribal isolated areas where specialized care is currently scarce; and - Ensure Long-Term Workforce Retention - Strengthen Arizona's rural health workforce by securing five-year service commitments from graduates who will practice in rural communities. Participating universities will provide career navigation and employment support to connect learners with rural healthcare opportunities and encourage long-term placement. This strategy can be implemented through two complementary pathways:
- Subsidized Learners: Students who receive financial support commit to fulfilling a five-year service obligation in eligible rural Arizona healthcare positions. - Non-Subsidized Learners: Students who do not receive financial support participate in targeted career services and employment outreach to increase awareness of and access to rural healthcare career opportunities, promoting voluntary placement in rural communities. We invite Arizona public universities to submit a Letter of Intent (LOI) to participate in this transformative annual investment in our state's future. While this request specifically seeks detailed plans and budget for Year 1 implementation, OEO is also open to reviewing proposals that incorporate a secondary Year 2 option to ensure project sustainability and continued growth. If the proposal incorporates a Year 2 option please also submit a separate Year 2 budget and implementation plan.
- Target Population: All activities must focus on healthcare professions in rural and tribal communities. Rurality is defined by Arizona's HRSA census tracts. Reference the ADHS GIS Map at: https://adhsgis.maps.arcgis.com. - Priority Impact: Successful applications must emphasize the ability to influence at least one of the following measurements: - Number of rural learners trained. - Number of workers placed in rural health systems. - Number of workers retained in rural health systems.

Solicitation # CMS-RHT-26-001
The Arizona Office of Economic Opportunity is implementing the Rural Health Transformation Program (RHTP) under solicitation CMS-RHT-26-001 to address healthcare workforce shortages in rural and Tribal communities. This cooperative agreement targets Arizona community colleges to expand Allied Health and Nursing professions through workforce training, educator support, and system coordination. The program emphasizes the training, placement, and retention of rural learners and requires any financial incentives provided to participants to be tied to a minimum five-year service commitment in a rural community. The initiative is structured over five budget periods from fiscal year 2026 through 2030, with an expected start date of December 31, 2025. Funding is divided into baseline and workload components, with workload distribution based on a points system evaluating rural factors and technical scores, such as Medicaid provider incentives and remote care services. Awardees must submit quarterly performance reports tracking specific metrics, including increases in the number of trained preceptors and rural high school students engaged in health professions. Applicants are required to maintain active SAM.gov registration and provide a Unique Entity Identifier. The application process requires a project narrative, budget narrative, and a Governor's endorsement, with a final submission deadline of November 5, 2025. Administrative and indirect costs are capped at 10 percent of the total award.
