The Aging & Disability Services and MnCHOICES Division within the Social Services Department would like to contract with agencies to provide case management services and care coordination for Ramsey County residents under the following programs: Alternative Care (AC), Brain Injury (BI) Waiver, Community Access for Disability Inclusion (CADI) Waiver, Community Alternative Care (CAC) Waiver, Developmental Disability (DD) Waiver, Elderly Waiver (EW), Essential Community Supports (ECS) Rule 185 Case Management and Vulnerable Adults/Developmental Disability Targeted Case Management (VA/DD TCM).
