AMENDMENT 1 TO COMBINED SYNPOSIS/SOLICITATION


This amendment is issued to implement the requirements of EO 14398, Addressing DEI Discrimination by Federal Contractors. FAR 52.222-90, Addressing DEI Discrimination by Federal Contractors (APR 2026) (DEVIATION APR 2026) applies to this acquisition.



AMENDMENT TO COMBINED SYNPOSIS/SOLICITATION


This amendment is issued to provide response to vendor inquiry. Attached is Attachment 4 - Brand Name Justification.



This is a combined synopsis/solicitation for commercial products prepared in accordance with RFO FAR Part 12. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued.



Solicitation No. 75H71326Q00030 has been issued as a Request for Quotation (RFQ) for a firm-fixed-price commercial product purchase order for a Dental Panoramic X-Ray Unit Replacement for the Indian Health Service, Portland Area Office, Western Oregon Service Unit, Salem, Oregon.



The scope of work includes one (1) brand name only Planmeca PROMAX 3D LEC 11X8 W/CALM ULD (Part No. CBVT-P-LEC-CALMULD) 3D panoramic, medium field-of-view cone beam computed tomography (CBCT) extraoral imaging unit, including freight, installation, software, software updates, remote support, and operator training. No substitutions will be accepted.



Delivery shall be made, FOB Destination, within 45 days after receipt of order. Installation shall be completed on site within 15 days after delivery. The Contractor shall provide one day of on-site operator training during Western Oregon Service Unit business hours after installation and functional verification.



The place of delivery, installation, training, and Government acceptance is the Western Oregon Service Unit campus, Chemawa Indian Health Center, 3750 Chemawa Road NE, Salem, Oregon 97305. Government acceptance will occur after delivery, installation, functional verification, and training are completed in accordance with the Specifications.

This requirement is set aside for Indian Small Business Economic Enterprises. The Contractor may submit an invoice through IPP after delivery, installation, functional verification, and operator training have been completed and accepted by the Government, unless otherwise authorized by the Contracting Officer. Invoices shall identify the applicable CLIN, unit price, total price, delivery date, installation date, training date, and any applicable serial number, warranty, remote support, and software update coverage information.



INSTRUCTIONS TO RESPONDENTS AND EVALUATION CRITERIA


Submission of Quotes


Quotations shall be submitted electronically to jacob.blalock@ihs.gov and pao.doa@ihs.gov no later than June 9, 2026 at 5:00 PM Pacific Time. The email subject line should read: 75H71326Q00030 - WOSU Dental Panoramic X-Ray Unit Quote.



The Government may consider a quotation received after the due date and time only if the Contracting Officer determines that doing so would not unduly delay the acquisition and would be in the Government interest. Offerors are responsible for ensuring timely receipt.



Quotes shall include:


  • Completed price quote (CLIN Schedule of Items) with a firm-fixed price for CLIN 0001, and shall include the dental panoramic x-ray unit, freight, delivery, installation, software, software updates, remote support, warranty, operator training, supplies, equipment, tools, materials, labor, travel, insurance, overhead, profit, and all other costs necessary to meet the Specifications.
  • Confirmation of active SAM.gov registration for Federal Government contracts and the offeror's UEI.
  • Evidence that the offeror is an authorized OEM reseller and that the installation technician and trainer are OEM-certified, or that such evidence will be available upon request before award.
  • Completed IEE Form.

BASIS FOR AWARD


The Government intends to award a purchase order to the responsible Offeror whose quotation conforms to the solicitation, meets the Specifications, and has the lowest total evaluated price.


The Government reserves the right to:


  • Make no award
  • Evaluate quotes without discussions
  • Communicate with Respondents for clarification purposes only
  • Exclude quotes that fail to follow solicitation instructions


EVALUATION FACTORS


The Government will evaluate quotations to determine whether the quoted product and required delivery, installation, software, support, warranty, and training meet the Specifications. The Government will determine whether each offeror is responsible and whether the total price for CLIN 0001 is fair and reasonable. Award will be made to the responsible offeror with the lowest total evaluated price among quotations that meet the Specifications. No tradeoff will be made.



AWARD INFORMATION


A single award is anticipated. Written notice of award or acceptance of a quotation furnished to the successful offeror within the time for acceptance specified in the quotation shall result in a binding contract without further action by either party. The Government reserves the right to award without exchanges, to communicate with offerors if needed, to reject any or all quotations, and to waive informalities and minor irregularities when in the Government interest.
