GENDEX GXDP-300 DIGITAL PANORAMIC X-RAY MACHINE

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is RFQP0302120009 and is issued as a Request For Quote (RFQ), unless otherwise indicated herein. The Federal Bureau of Prisons, FCC Coleman, Coleman FL 33521, intends to make award for the following: 1. Gendex GXDP-300 Digital Panoramic X-Ray System (see attachment for specifications) 2. GXDP-300 Free Standing Base 3. Installation The Federal Bureau of Prisons requires the following items as Brand Name or Equal. New Equipment ONLY; NO remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. This solicitation is unrestricted, FOB Destination. The contract type is firm-fixed price, using Simplified Acquisition Procedures. The Government will award to the responsible offer whose offer, conforming to the solicitation that is considered to be the most advantageous to the Government, with past performance and price considered. Interested parties are responsible for monitoring this site to ensure having the most up to date information about this solicitation. The closing date for this solicitation is May 14, 2012, by noon, Eastern Time. Contractor must be registered on Central Contract Registration (CCR) @ www.ccr.gov. Payments will be made using Electronic Funds Transfer (EFT). Shipping must be free on board (FOB) destination CONUS (Continental U.S.), which means that the seller must deliver the goods on its conveyance at the destination specified by the buyer, and the seller is responsible for the cost of shipping and risk of loss prior to actual delivery at the specified destination. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5 Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, Clause 52.228-5 Insurance - Work On A Government Installation (JAN 1997); Clause 52.237-2 Protection Of Government Buildings, Equipment, and Vegetation (APR 1984); and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34. The following requirements apply to those individuals conducting installation inside the institution: 1. National Crime Information Center (NCIC) Check (attached) 2. DOJ Contractor Residency Requirement Certification (attached) Any individual who does not pass the security clearances will be unable to work within the secure perimeter of the institution. The final determination and completion of the security investigation procedures will be made at the sole discretion of FCC Coleman.
