Resolve will build on the foundation established during Phase 1 and will focus on developing / refining a comprehensive LFC model and evaluation plan, both of which will be tested in pilot communities. The LFC Model development process will occur over several months with the goal to gather input from both internal and external experts. Resolve will assist with facilitating the development of both the LFC Model and draft evaluation plan to assess the impact and evaluation of the LFC Model. Resolve will assist the CDC in refining the LFC Model and evaluation plan based on the feedback gathered.
