Rural Health-Medicaid Primary Accountable Care Transformation (PACT)

1.1.1 Background: The State of South Dakota Department of Social Services (DSS), Division of Medical Services (DMS), is committed to transforming rural healthcare through the Rural Health Transformation (RHT) initiative - a statewide effort focused on strengthening rural, Tribal and frontier healthcare systems and improving access to quality care through digital health modernization and coordinated care delivery across South Dakota. 1.1.2 Goals and Objectives: Central to this initiative is the Medicaid Primary Accountable Care Transformation (PACT) program that promotes shared accountability for cost, quality and health outcomes among Medicaid enrolled primary care clinics, Rural Health Clinics (RHCs), Federally Qualified Health Centers (FQHCs) and Tribal partners and to build a more resilient, connected, and sustainable rural health system for communities across the state. 1.1.3 Description of Components or Phases: To operationalize the Medicaid Primary Accountable Care Transformation (PACT) model, The State of South Dakota is issuing this Request for Proposal (RFP) to solicit proposals from qualified vendor(s) or offeror(s) for the procurement and implementation of a Case Management Tool. This tool will provide a core technology capability that supports care coordinators, case managers, and clinical teams in delivering coordinated, person-centered services for applicable South Dakota Medicaid members. It will also be utilized for juveniles eligible under section 5121, CAA 2023 for up to 90 days of targeted case management and referrals to community-based care and services. 1.1.4 Scope of Components or Phases: See Section 3 - Scope of Work for more details. Offerors may email inquiries concerning this RFP to obtain clarification of requirements. No inquiries will be accepted after the date and time indicated in the Schedule of Activities. Inquiries must be emailed to Kirsten Blachford at Kirsten.Blachford@state.sd.us with the subject line “RFP # 27-09RHT-026”. The State will to respond to offeror’s inquiries (if required) via e-mail. In addition, all inquiries and the State’s response will be posted on the state’s e-procurement system. Offerors may not rely on any other statements, either of a written or oral nature, that alter any specification or other term or condition of this RFP. Offerors will be notified in the same manner as indicated above regarding any modifications to this RFP. Respondents are encouraged to carefully review the RFP in its entirety before submitting questions. Questions should be limited to those necessary to clarify the requirements of the RFP or information needed to prepare a responsive proposal. Respondents should avoid submitting duplicative, hypothetical, overly broad, or excessive questions. Questions that can be answered through a reasonable review of the RFP and its attachments should not be submitted. The State also discourages the submission of bulk or automatically generated questions, including questions produced through artificial intelligence (AI) tools without meaningful respondent review. Respondents are expected to review, consolidate, and prioritize questions prior to submission to ensure each question is specific, relevant, and necessary to clarify the RFP requirements.
