Full Description
PURPOSE OF AMENDMENT
The purpose of this amendment is to:
- Revise Attachment D – Price Schedule to provide a clear and consistent pricing structure for the requirements identified in the Statement of Work (SOW);
- Provide separate pricing elements for Bulk Liquid Medical-Grade Oxygen, routine oxygen delivery, H-Size reserve oxygen cylinder rental, Oxygen cylinders, Cylinder Delivery Charge, Annual Bulk and Reserve System Gauge Verification, and Emergency Medical-Grade Liquid Bulk Oxygen Delivery and Hookup;
- Align the Price Schedule with the estimated annual Bulk Medical-Grade Liquid Oxygen usage identified in the SOW;
- Establish a pricing mechanism for the Emergency Medical-Grade Liquid Bulk Oxygen Delivery Service already required by the SOW; and
- Request submission of a complete revised quotation using the revised Price Schedule.
- The following has also been revised and updated: Attachment B - Clauses and Provisions and Attachment C - Statement of Work to only change the CCF quantities. SEE ATTACHMENTS.
This amendment does not remove the requirement for Emergency Medical-Grade Liquid Bulk Oxygen Delivery Service contained in the SOW.
