NOTICE OF INTENT TO SOLE SOURCE The U.S. Government currently intends to award a supply contract to provide Salient features for 5 Trophon2 Ultrasonic cleaning systems on an SOLE SOURCE basis for the Department of Radiology for Trophon2 ultrasound probe disinfection unit at Bayne-Jones Community Hospital (BJAHC) at Fort Polk, LA. Period of Performance: Base: 01 October 2021 - 30 September 2026 with four (4) twelve month option years The proposed contract number will be awarded on a sole source Firm-Fixed Price (FFP) action to: Endoscopy MD, LLC 2342 Baltimore Pike Oxford, PA 19363 Description of Services or Supplies: is for five N0500-US Trophon 2 ultrasound probe disinfection units, five N00048NNA Trophon 2 printers, five N00049-ROW reams of printer paper, and five boxes of N00102 Trophon 2 clean probe covers, SERVP048 four year extended warranty and SERVP052 one year extended warranty. This requirement is for If your organization has the capacity and authority to provide, 5 Trophon2 Ultrasonic Cleaning Systems that are to be used to sterilize ultrasound probes, with printer, warranty, service contract, shipping and any freight chargers. - Printer - Training seminar - Accessories, such as printer paper, printer cable, clean covers - 7 year service agreement with loaner equipment available - Trade in value for current systems Five Trophon 2 units and accompanying printers, paper, and probe covers, will be delivered to Bayne-Jones Army community Hospital. A one day training seminar to educate super users from radiology and labor and delivery will be needed to comply with infection control and The Joint Commission standards. A five year extended service agreement is requested. The extended service agreement will cover for four years Annual PM with loaners; 5000 cycle refresh with loaner; breaking/fixing parts with loaners; and all ground transportation. An additional year extended service agreement will cover annual PM with loaner and all ground transportation. This sole source requirement is issued under the statutory authority of FAR 13.106-1(b)(1): For purchases not exceeding the simplified acquisition threshold (SAT), only one source reasonably available, urgency, exclusive licensing agreements, brand name, or industrial mobilization.
