26-64023: Onsite/Offsite Oral and Maxillofacial Services

The California Department of State Hospitals is seeking qualified providers to deliver onsite and offsite oral and maxillofacial surgical and dental services on an as-needed basis under a contract anticipated to run from July 16, 2026, through July 15, 2029. The contractor must supply all personnel, materials, equipment, transportation, licenses, permits, insurance, and certifications necessary to perform these services at DSH-Coalinga and other contracted medical facilities across California, ensuring that all clinical evaluations and documentation meet the stringent standards of Title 22 of the California Code of Regulations, the Joint Commission, and the Department of Justice. Services are to be provided by licensed dentists or groups of dentists holding current licenses from the Dental Board of California and board certification from the American Board of Oral and Maxillofacial Surgery, with the contractor required to be located within a 90-mile radius of Coalinga. The contract operates under a fixed-rate schedule for specific dental procedures, including extractions, surgical removals of impacted teeth, dental consultations, anesthesia, and imaging, with all rates binding for the term of the agreement. The total contract value is capped at $675,000, with pricing inclusive of all supplies, medications, and materials. Bidders must submit a complete and sealed proposal, including mandatory attachments such as the Bid Submission Checklist, Bidder’s Certification, Minimum Qualifications Response, Rate Sheet, Insurance Certifications, Contractor Certification Clauses (CCC 04/2017), DVBE Declaration, and other compliance forms, all requiring original signatures.
