Please be sure to include the state contract number in the contract column of the grid, if applicable.Completion of the grid is required and will be considered as part of the response for evaluation. Completion of the questionnare is required and will be considered as part of the response for evaluation. If a State contract number is not provided, the State Standard Terms and Conditions apply to this solicitation. No additional terms and conditions will be accepted.Selection will be based on the lowest responsive and responsible quote.All responders to this solicitation must be fully registered as a Supplier to do business with the State of Ohio. Please follow all steps in the RFQ to be fully registered.At its sole discretion, DAS may renew the contract for up to two additional one-year term(s) subject to the satisfactory performance of the selected supplier and the needs of the State.
