The Northern Arizona VA Healthcare System, along with its Community-Based Outpatient Clinics and affiliated clinics, is seeking a contractor to provide 24/7 non-emergency ground ambulance transportation services for both Basic Life Support and Advanced Life Support needs across multiple locations in Arizona, including Prescott, Flagstaff, Cottonwood, and Anthem. The contractor must supply all vehicles, personnel, equipment, management, and reporting necessary to meet demand, with the ability to handle up to two simultaneous transports at any time. Services must be available without interruption, including weekends and holidays. Personnel must be qualified with at least 150 hours of training meeting Department of Health and Human Services standards, including an in-hospital component, and all drivers must hold valid state-issued licenses. Medical staff must possess current certifications from recognized authorities, and the contractor must hold an Arizona State Certificate of Necessity issued by the Arizona Department of Health Services and Bureau of Emergency Medical Services.
