AMENDMENT 0003 Onsite Phlebotomy Services- Houston, TX

This contract solicitation, issued under solicitation number 36C25626Q0476, seeks qualified Service-Disabled Veteran-Owned Small Businesses to provide onsite phlebotomy services at the Michael E. DeBakey VA Medical Center in Houston, Texas. The requirement is for a five-year indefinite-delivery, indefinite-quantity (IDIQ) fixed-price contract with annual ordering periods, each beginning on the award date and lasting twelve months, with an option to extend performance by up to six months total.
