Beale AFB Mortuary Affairs

The U.S. Air Force, through the 9th Force Support Squadron at Beale Air Force Base, is soliciting offers to establish a Blanket Purchase Agreement for mortuary affairs services in Linda, California, covering all of Yuba and Sutter counties. This requirement is a Total Small Business Set-Aside under FAR 19.5 with a NAICS code of 812210, meaning only small businesses are eligible to respond. Offerors must be licensed funeral establishments in good standing with state and federal regulations, maintain an active SAM.gov registration, and comply with all applicable local, state, and federal licensing and regulatory standards. The contractor is responsible for providing a full range of mortuary services including embalming, restoration, preparation, care, and disposition of remains for authorized personnel, along with necessary supplies such as caskets, urns, death certificates, and shipping containers that meet air carrier and destination country specifications. Cremation services are restricted exclusively to subsequent portions or retained organs, and facilities must be licensed solely for human remains—no pet cremation may occur in the same building. All services must be performed within 24 hours of notification or release of remains, and operations must adhere to a 3:00 p.m. local time cutoff for remains release imposed by the local coroner. The contractor must maintain inspected and certified facilities, equipment, and motor vehicles, employing universal precautions and hygienic practices in compliance with OSHA standards. All remains must be properly tagged at the foot end with the deceased’s name, rank, last four digits of their Social Security Number or CAC, and date of death. Documentation, including DD Form 2063 for CONUS and DD Form 2062 for OCONUS, must be completed and submitted as required. Quality assurance is governed by a comprehensive Surveillance Plan requiring 100% inspection of facilities, personnel, caskets, urns, shipping containers, and documentation, with additional inspections conducted until deficiencies are resolved. The contractor must also maintain an internal inspection system with documented corrective actions available to the government.
